Letterhead: "Dalyte Electric Limited / MANUFACTURERS OF "Dalyte" NITROGEN LAMPS, "Dalyte" TUNGSTEN LAMPS, "Kraft Cord" FLEXIBLE CONDUIT". In the lower left corner is the company logo - a sketch of a factory with a smokestack. Letter notes:
"Arrangements have been made whereby all employees of the company may obtain Life Insurance upon a co-operative basis. The amount which each employee may obtain is $500 for Females and $1,000 for Male employees, regardless of age or physical condition. This insurance may be obtained without medical examination at a cost to each employee of .60c. per month per $1,000 insurance. Payment will be deducted once each month. THIS COMPANY WILL PAY THE BALANCE OF THE PREMIUM.
This policy provides for payment of the full amount of insurance in the event of death at any time, any age, any place and from any cause whatsoever, or if an insured employee should become permanently and totally disabled before age 60, the full amount will be paid at once. Each employee who subscribes to this insurance will receive an individual certificate as evidence that you are insured. The insurance will remain in force as long as you stay in the employ of this company and continue to pay your share of the cost. However, if any individual should leave our employ and desires to continue the insurance, he or she may do so by changing the insurance into any kind of policy issued by the Aetna Life Insurance Company, without medical examination, within 31 days' time after leaving. The cost to change this insurance will depend on the age at time of such conversion and also on the plan of insurance adopted at that time.
Those employees who desire to take advantage of this opportunity of obtaining insurance under such favourable conditions may do so by filling in the attached application and turning same into the office immediately. This plan is not compulsory but it is believed by us to be a very advantageous arrangement and it is hoped all will take advantage of it. Very truly yours, Sec'y-Treas."
