White paper certificate with blue printing. The left hand side of the sheet has a vertical row of seven stickers listing dates from 1965-1970. The top has the Ontario logo beside thick letters "DEPARTMENT OF LABOUR, ONTARIO" Underneath in smaller letters is "THE APPRENTICESHIP AND TRADESMEN'S QUALIFICATION ACT, 1964" and again in thick letters "CERTIFICATE OF QUALIFICATION" The centre of the certificate has "This is to certify that ____ having complied with The Apprenticeship and Tradesmen's Qualification Act, 1964, and regulations is issued this certificate of qualification in the trade of MOTOR VEHICLE REPAIRER - A" (On the ____ "ORNAN BAER," is typed.) To the left of this is a blue, Department of Labour seal. The signatures of the Minister of Labour and the Director of the apprenticeship branch are in blue. "SIGNATURE OF HOLDER ______ DATE ISSUED ____ CERTIFICATE NO. _____ " are at the bottom. Ornan Baer's signature has been written in and "Nov. 26, 1964 611" have been typed in their appropriate spots. The Allied Printing Company's logo is near the bottom, between the stickers and labour seal and at the very bottom, in blue is "This Certificate must be renewed annually on or before the 31st day of October of the year following the issue of last renewal of the certificate." There are pinholes in the two top corners and in the centre on the bottom.
