Includes an order of service for Dedication service and a brief history of St. Andrew's. Rectangular off-white glossy paper, folded twice and printed on both sides in black lettering. Folded to form a booklet of six pages.
Front cover has b/w sketch of St. Andrew's with text above and below picture.
