Yellow covered booklet of Chalmers Church Annual Report.
Cove has a printed drawing of the Church in the front center. Titled above the picture is "UNITED CHURCH OF CANDADA" in a clear black font. Beneath the title continues in a black Gothic font "CHALMERS CHURCH GUELPH ONTARIO 1868 - 1966." Under the picture in a bold black font is "Annual Report for 1965."
The report is bound together by two staples seen on the spine.
On the inside of the front cover is a picture of the Church's Quebec Street Entrance.
